Welfare Fund open enrollment materials were mailed to qualifying participants on May 30, 2019 for coverage beginning July 1, 2019 through June 30, 2020.  Eligibility is based on your covered earnings during calendar year 2018.  Please see the "Eligibility and Enrollment" page under the Welfare section for more information.

Enrollment Forms due to be returned to the Fund Office by

June 17. If you have not returned your form and wish to elect coverage, please call Gloria at (212)247-5225, ext 104 as soon as possible!  Self-pay premiums not received by JULY 1 will have a $25 late payment penalty assessment.

If you are currently enrolled for medical coverage, you do not need to fill out the Enrollment Form unless you wish to add or drop eligible dependents, need to change your address, or wish to “Buy Up” to Tier III coverage.  (However, please do note the self-pay rates on the enrollment form and read the information regarding the self-pay Wellness Incentive.)  Otherwise, you will be receiving an invoice for your self-pay premium, which after being paid, will automatically enroll you for continued coverage during the next Plan year, July 1, 2019 to June 30, 2020.  However, if we do not receive your self-pay premium, your coverage will be terminated as of June 30, 2019 and you will not have another opportunity to enroll for coverage for the period July 1, 2019 to June 30, 2020.

PLEASE NOTE that if you were previously eligible for benefits but either did not enroll or lost your coverage for any reason, you MUST complete the Enrollment Form in order to participate during the coming year.

New Participants:

Please read and review the enrollment materials and complete the Enrollment Form carefully. The Enrollment Form lists the self-pay rates for the different types of coverage, and provides space for you to elect coverage for dependents.  You MUST provide a marriage certificate, children’s birth certificates, and copies of social security cards for all listed dependents or we will not be able to process your enrollment form. 

 

PLEASE NOTE that the Enrollment Form and your Payment must be returned to the Fund Office by June 17, 2019 to ensure that your coverage is in place on July 1, and that you receive your Aetna and Express Scripts ID cards on time.  You must include your first self-pay premium payment, and the required documents for any dependents as listed above.
