FUND OFFICE NOW ACCEPTS

CREDIT CARDS 

The Fund Office is now accepting self-pay premium payments with credit cards. Please note the following guidelines:

1. Each credit card transaction will require payment of a
$5.00 processing fee. This fee will be added to your credit card transaction total.

2. Only Visa and MasterCard credit cards are accepted. We do not accept American Express or Discover.

3. Credit card payments may be made in person at the Fund Office, or over the phone by calling (212)247-5225.

4. We are only permitted to accept payments for one quarter of health coverage. If you would like to pre-authorize payments by credit card for the remainder of the year, you will need to sign an authorization form which you can download HERE.

5. Any overpayment will be returned to you by check. No credits or refunds will be issued to your credit card.

6. Credit card payments are subject to the same deadlines and penalties as payments made by cash or check.

7. No one may make a credit card payment on your behalf. We will only accept payments for your coverage on your credit card.

8. Payments by credit card must be for the full amount of the quarterly self-pay premium, including any applicable late penalty or other fees. Partial credit card payments are not permitted.
